Your First Visit
Upon arrival at the clinic, you will be asked to complete health information forms. If you would like to fill out these forms prior to your appointment, please pick up a copy of the paperwork at Plaza Internal Medicine, William L. Bradley Medical Plaza, 3 Appleby Road, Fayetteville, AR 72703.
During your first visit, Dr. Radkey will review your health history and information you provide. Your visit will include a physical exam that allows Dr. Radkey to establish your general health condition. If you have medical records available from previous physicians, please have them forwarded to our office so that they may be received prior to your visit.
Please note: In order to devote full attention to your most important health issues, Dr. Radkey will focus on your primary health concerns during your appointment. For patients with multiple health concerns, additional appointments may be necessary to adequately address additional health issues.
For patients with acute illnesses such as cold, infection or flu, Dr. Radkey will perform a brief examination to determine the extent of your illness and you will be treated promptly.
Patients seeing Dr. Radkey for wellness checkups, please be aware these appointments may require additional time for a thorough assessment. In order to allow sufficient time for your appointment and ensure proper insurance filing, please inform the appointment desk when scheduling your wellness checkup.
For patients seeing Dr. Radkey for the treatment of chronic diseases and conditions (e.g., diabetes, hypertension, and thyroid disorders), appointments may require labs as well as thorough examinations to monitor your condition and general health.

Please arrive 15-20 minutes prior to your scheduled appointment time to complete the registration process. Please have your current insurance card; your insurance card will be requested at each visit.
Please bring all your prescription medications and supplements with you to your appointment.
If you need to cancel or reschedule your appointment, please provide as much notice as possible.
